ASTHMA

Forms for Children with Asthma

Asthma Action Card lets staff know what to do in the case of an asthma episode. This is a sample form produced by the U.S. Environmental Protection Agency. Especially now as the incidence of asthma is increasing and a teacher may have more than one child with asthma in the classroom, it is crucial to be able to instantly check what is the proper treatment for that child.
Daily Asthma Management plan, is a form, also from the U.S. EPA that provides information such as what triggers an asthma episode and what children have been instructed to do by their parents.

DIAGNOSIS

Suspected Disability Form

Observation report – one of the recommendations when a teacher or other staff member suspects a child has a disability is to write down observations. This form can be used to record observations of a child with a suspected disability. It will be especially useful to new staff members who are unsure of the type of information they should record. It is also useful to old, experienced staff members who plenty busy and don’t have the time to create their own form!

FEEDING, FOOD and NUTRITION

The Food Pyramid, a simplified color poster for use in early childhood classrooms and primary grades.
The Food Pyramid coloring worksheet, can be printed out for nutrition lessons in pre-K through three, and also for children with special needs.

 Individualized Education Plan (IEP) Forms

Put these forms together and you have a very good start to writing a good IEP.

Accommodations and modifications: a two-page form that is a written agreement specifying what accommodations and modifications will be provided in the classroom.
IEP Goals and Objectives - a form for documenting goals, objectives and progress toward meeting each.
IEP addendum – this form is used to document changes to the IEP that occur after the initial IEP meeting.
Pupil Placement Summary - this form accompanies the IEP and summarizes important information about the student's placement in special education, such as parent involvement, how much time the student will spend with students without disabilities, the school and district where the students receives services and placement options discussed.
Special Education periodic communication form – this is a form that can be used by special education staff to receive regular reports on the progress of students with special needs in the regular classroom. It would most often be used by a resource specialist or learning disabilities teacher.

Individualized Family Service Plan (IFSP) Forms

Sample form from Alaska - this form is very similar to what is used in most North Dakota programs
Sample IFSP form from Nevada - which you might think would not be necessary in the Great Plains. However , the growth rate of the population of Spanish speakers outside the traditional southwest and northeast regions.

Individual Transition Plan (ITP) Forms 

These two forms taken together will be extremely useful to anyone creating an Individual Transition Plan for a student.

Individual Transition Plan form – includes areas for all required areas of the transition plan as well as some optional areas.

Individual Transition Plan documents – this is a form that can be included with an Individual Education Plan (IEP). It gives the student’s/ family’s preferences for post-school outcomes, such as independent living or living with the family, it specifies what courses the student has already completed that will aid in getting employment. In general it summarizes most useful information for getting a job. My one complaint about this form is that, although it is very good, it is focused only on employment and living in the community. It does not address the needs of students who intend to go on to college.
